In-Home Service Authorization & Risk Acknowledgment

Many appliance repairs require moving, disconnecting, and reconnecting the unit.

When appliances are moved, older plumbing lines, valves, drain hoses, electrical outlets, flooring, cabinetry, or shut-off valves may fail if they are worn, brittle, corroded, improperly installed, or previously stressed.

This form is required before we dispatch a technician.

Normal service procedures include reasonable movement, disconnection, reconnection, and testing of the appliance.

By signing below, I acknowledge and agree to the above terms and release TN Appliance Exchange from liability related to pre-existing, concealed, deteriorated, or improperly installed conditions that fail during normal service procedures, including any consequential, secondary, or indirect damage arising from such failure.
